3-minute self-check
Is your home survey-ready?
Your caregivers give good care every day. The question a surveyor asks is whether the chart shows it. Answer ten plain questions about how your home charts today. Your score and where to look first show up right away. No email needed.
1. Could you show a surveyor a note for every resident, from every shift, this past week?
2. When a caregiver notices a change in a resident, is it written down the same shift?
3. After a change of condition, is the call to the resident's practitioner written down?
4. And is the call to the family or representative written down?
5. After every fall, are both the practitioner and the family notified promptly, and is that written down?
6. Do your notes describe what actually happened, instead of labels like "agitated" or "combative"?
7. When a resident refuses care or medication, do the notes show what staff tried and how the resident responded?
8. Is every incident report reviewed by the administrator within a few days?
9. Is every grievance acknowledged within 3 business days and resolved within 30 days, on paper?
10. If a caregiver called in sick tomorrow, could the next caregiver learn everything important from the chart?
0 of 10 answered. Your score appears when you finish.
